Life Changes Care Management™ was created with a commitment to advocating for person-centered and respectful care for older adult clients. Donna has used her decades of clinical nursing experience to provide compassionate and highly customized support to older adult care receivers and their caregivers. The practice now includes Ali Pethe, a Licensed Social Worker. With two Professional Aging Life Care Managers®, we’ve formed a highly collaborative partnership. We enjoy incorporating our strengths and different perspectives to ensure our clients receive the support they deserve.
Donna Farley Forrest, RN, CHPN, CCM, CDP
As the owner and creator of Life Changes Care Management, I bring over 30 years of experience as a Registered Nurse. When you work with me you gain immediate access to my full range of knowledge and expertise. I value building positive collaborative relationships in all walks of life. I step into your world and working together, we create the best solutions to your most challenging concerns. My recommendations are based on the desires of the care receiver and the personal strengths of the caregiver.

A core passion of mine is excellent care for individuals with serious and complex health conditions and those at the end-of-life. I enjoy supporting those living with illness at any stage and their loved ones to live well each and every day. As a clinical manager of an in-patient hospice unit, I was privileged to train my staff with this vision of care.
Advance Care Planning – Facilitating life-changing conversations with my clients, their loved ones, and their healthcare providers bring me deep personal satisfaction. This ensures that my client’s desires for care and treatment are heard and respected through the use of Advance Directives.

Another passion is respectful and compassionate care for individuals living with dementia. I’ve been a dementia practitioner for many years across numerous settings including skilled nursing facilities, physician’s offices, home care, and a facilitator of Alzheimer’s support groups. My role as director of health services in assisted living and memory care allowed me to serve many individuals and their families in this care setting. This is why I am a strong supporter of, and Dementia Friendly Leader® with Dementia Friendly Communities of Northern Colorado®.

I bring over five years of experience working in many systems that serve older adults including nursing facility, non-profit, and hospital environments. I love using my knowledge and expertise to partner with caregivers and collaboratively create a plan of action to meet the unique needs of your caregiving situation. By sharing my knowledge, I hope to empower you to navigate these systems and ease the experience of caregiving.
I assist with all aspects of the aging journey. I am particularly interested in:
Dementia Care: Caring for people with dementia should be about the person, not the disease. I’m very focused on improving the care and quality of life for people living with dementia. I love sharing new techniques and perspectives with caregivers that I’ve learned from experts in dementia care.
End of Life Care and Planning: I believe dying is one of the two most significant experiences we have as humans (being born is the other). I have significant experience working with, and connecting families to, Hospice and Palliative care teams to improve comfort and ease the transition from this life.
Advance Care Planning: A core passion of mine is assisting people to plan for their end-of-life care by outlining their wishes in Advance Care Plans. I believe completing the documents to clearly state these wishes is one of the greatest gifts we can give our loved ones.
